hours, or by leaving a message on voice mail. Your
call will be returned within 24-48 hours. Every
effort will be made to accommodate your schedule.

| Cancellations |

notify the office at least 24 hours in advance should
you need to cancel an appointment. You will be billed
for missed appointments not cancelled within the
time frame. |

medical records are held in strictest confidence
and will not be released to anyone without your
written permission. |

| Insurance
and Billing |

| Payment
is required at the time of service. After your visit,
you will be provided with an itemized receipt, which
you can submit to your insurance carrier. |
